Greetings from the new office space! I'm excited about the response I'm getting from patients and I'm glad for the questions. As this is a new enterprise, there are things I didn't foresee that need some clarification regarding the enrollment process for Imago Dei. Hopefully this helps! If you have other questions, please do not hesitate to reach out.
1. You may enroll as a family, or you may enroll as many of your family members as you wish. Many families are enrolling only their children, or some of the children.
2. The Electronic System I'm using allows for only one type of enrollment in the sign-up process and that is the monthly membership plan. However, those who are enrolling as non-membership patients still need to go through this same process as non-members. Please let me know if this is you so I can change this on your patient profile and adjust your billing cycle. If you have questions about the difference in membership/non-membership options, please consult the website. If you still have questions, please reach out to me.
3. There is a family cap at $300/month. There is no co-pay for an office visit. That's it!
4. The Patient Agreement is available to see and required for you to sign during the enrollment process. However, if you would like to see the Agreement prior, please send an email and I'd be happy to send it to you.
5. While I'm not accepting Medicaid or Medicare, I will remain an ordering, prescribing, and referring physician. That means you can still submit bills for labs, imaging, etc. (depending on your plan). There will be forthcoming information about Medicare in the future. For example, did you know that parts B, C, and D are optional?
